is there a specific alkaloid that the user becomes hooked on? i know endorphins naturally created in the brain that act as the reward system, are in essence opiates, because they act on the same receptors. are we addicted to them too than?
It depends on what form the opiate is in that the person is taking.
There are four addictive alkaloids in opiates: Morphine, codeine, thebaine and papaverine. Both street drugs and prescription drugs containing opiates can have one or more of the addictive alkaloids present. These alkaloids bind to certain opioid receptors in the brain, and this can lead to dependence.
Endorphins do help us "feel good", but the relationship to opiates is inexact. The matter of endorphins and the opiates is a tricky one that can result in confusion and misunderstandings. It's best to discuss with a physician who specializes in treating people with addictions.

I think that it involves acupunture needles and the machine periodically fires mild electric implulses to various points in the ear which somehow simulates the euphoric part of the opium high. What is this thing?

doesn't the pharmacy realize there is something wrong if the person keeps coming back for the pills?
You would be surprised how creative someone is when they need their pills!
I have a family member that is addicted to pain pills. They go to about a dozen different doctors and use just as many pharmacies to get the pills filled.
They have a note pad with all information in it--what pharmacy to use with what doctor, rather it they can use their insurance or have to pay out of pocket,ect.
You would think jail time would have opened their eyes but it hasn't.
Some pharmacies do catch on--more like the "mom and pop" pharmacies.
I know Walmart red flags your account when you have a lot of rx's for pain meds filled--I ahd that happen one time when I was going threw soem major dental problems.
It was flagged because two different dentist were writing the rx's but once they knew that each dentist knew about the other and what I was taking, it was fine...one was my regular dentist, the other was a speicalist.

lets say a person used to be an opiate addict i mean along the lines of vicodin and oxycontin etc.
and lets say he ends up injured or ends up needing surgey and is prescribed pain meds and is necessery.
what would an ex addict do if he could easily relapse that way?
Having been in that situation, I speak from experiance.
Two choices:
1) Try to be tough enough to get by with over the counter meds. I tried Advil when I was in the hospital. It did me no good. So, I went to option 2.
2) Take the opiates. By the time I was taking opiates, it wasn't about getting a buzz. I was in pain, and I needed some relief. Yeah, it was hard on me, mentally. But laying around in pain is no fun either. Sometimes, you gotta make a hard choice.
my husband has been addicted to opiates for a few years. he has been clean for nearly a month, but still needs guidance. its really taking a toll on our marriage, so i want to go to the counseling as well and learn what we need to do as a family to make things better. i dont know where to start though. what kind of therapist is needed?!
A counselor experienced with addictions. Also, you want a counselor that has experience working with couples. You can try asking your GP whether he knows any good counselors with experience in those things in your local area. Joining AA and I think it's called Al-Anon might be useful too. If I understand correctly they're not just for alcoholics but for other addictions as well.
